The U.S. Playbook to Address Social Determinants of Health

  • County Health Rankings and Roadmaps - The annual rankings reveal how health is influenced by where we live, learn, work, and play. They provide a starting point for change in communities.

    Social Interventions Research and Evaluation Network - Providing the latest on social intervention research and evaluation with a focus on improving health and health equity. 

    Translational Science Spectrum - This spectrum represents each stage of research along the path from the biological basis of health and disease to interventions that improve the health of individuals and the public.

    Community Based Solutions to Promote Health Equity in the United States - RWJF asked the Health and Medicine Division of the National Academies of Sciences, Engineering, and Medicine to help delineate the causes of and the solutions to health inequities in the United States. This report focuses on what communities can do to promote health equity and on the broader policy context and contributions of stakeholders that can support communities.

    Ten Essential Public Health Services - These social determinants of health (SDOH), and actions to address the resulting health inequities, can be incorporated throughout all aspects of public health work.

    Society Health VCU – Life Expectancy Map - The aim of these maps is to serve as a resource—raising awareness of factors that shape health and spurring discussion and action on a complex web of factors that influence health.

    Horizon Neighbors in Health - Launched in the spring of 2020 at the height of the pandemic, Horizon Neighbors in Health is the largest, most comprehensive program ever created in New Jersey to address social determinants of health. Horizon BCBSNJ, the state’s largest health insurer, is investing $25 million and partnering with some of New Jersey’s most respected health organizations over the next three years on a model that employs local Community Health Workers to connect members with a wide array of services to address challenges hampering their health and well-being.

    New Jersey State of Affordable Rental Housing - (NJSOARH) project seeks to understand the state of rental housing affordable to low-income and especially very low-income households in New Jersey and the processes that shape housing insecurity.

    New Jersey Office of the Food Security Advocate - The New Jersey Office of the Food Security Advocate (OFSA) is a convener and collaborator building consensus across resident leaders, "food and" pantries, community kitchens, community-based organizations, the six state-designated food banks, legislators, state agencies, agriculture, food rescuers, and others to make true food security a reality for everyone.
